Who Qualifies for a home visit?

Elite Primary Care & Wellness serves patients ages 16 to 100-with services such sick calls for a sore throat up to chronic complex care management for a variety of conditions. Our patient population includes those who are unable or are too sick to travel to the doctor, patients who live in rural areas, or those patients who are simply wishing to avoided a crowded conventional waiting room. Just call our office to get started as a new patient!

Are your services covered by my insurance?

Your insurance provider should cover the services of our providers as it would any other primary care services. We accept Medicare assignments and supplements. We also accept certain types of commercial insurances. 
We will file your claim for you with any accepted carrier; otherwise, we will give you a receipt upon completion of the visit that you may then file with your health insurer for reimbursement. 
If we are not a participating provider of your health insurance, we can offer our self pay price which varies upon visit type. You can pay via credit card or cash at time of service. There is also an additional convenience fee (for self pay patients) for evening, weekend, and holiday visits

How quickly can I schedule a home visit?

At Elite, we strive for same day appointments when available. We aim to schedule visits same day if not 24-48 hours from when you call (during the week). However, there are some exceptions, so please call us to set up an exact appointment time.

Can Elite Primary Care & Wellness become my primary care provider?

Yes. Primary care is our specialty, allowing for full service, in-home medical care and treatment.

What hours are available?

Standard office/visit hours are Monday through Thursday, 9am – 5 pm. Evenings, weekends, and holiday visits are also available based on patient need and provider availability (convenience fees may apply for non-Medicare patients).

Do you come out for emergency visits?

We do not make emergency visits. If it’s an emergency, call 911 or go to the nearest emergency room. We are not a 911 service.

What if I need an urgent house call?

Our mobile healthcare services prevent many urgent needs by seeing patients on an appropriate follow-up basis. When needed, we can usually get to your home for urgent needs with 24-48 hours.
